I'm happy this is going out - big improvements.

A couple of small nits about some XEPs:

* XEP-0198 (stream management) has reliable transport - i.e. any dropped
packets will be retransmitted when reconnecting.  It requires server
support.
* XEP-0184 (delivery confirms) requires support from the peer rather
than the server.
