I wonder if we can implement this form aspect using something like
OpenDataKit, and then just export the data from that into InformaCam? I
really don't like the idea of building such a complex "forms" aspect of
the app.

You can design a form here: http://opendatakit.org/use/build/

Then use the ODK Collect app to use it. Data is stored right onto the
SDCard, so I think you could easily import it into InformaCam from there.
